It should be noted that the 410 is a shaped device and as such it has a textured surface. A sand paper texture if you will. The benefits behind this texturing include a lower incidence of capsular contraction and tissue adherence to the shell resulting in less potential for bottoming out (excessive stretching of the lower pole breast tissues). However, tissue adherence is not 100% and as such if tissue adherence fails to occur after placement the implant may rotate out of its position postoperatively. If this occurs there is a very obvious deformity to the breast, which will necessitate revision surgery and all of its associated expenses. Prevention of this complication starts with a pocket that is precisely dissected so that the implant fits into the pocket like a hand in glove. Too big a pocket – problem, too small a pocket – problem.

Well after a long wait the Allergan 410 has arrived. Also frequently referred to, as the ‘gummy bear’ implant the 410 is the first of its kind when it comes to form stable implants. The silicone gel found in this implant is cohesive enough that when cut in half the contents do not pour out from the implant shell like a hot berry pie. Instead they are retained within the confines of the shell – much like a freshly cut piece of lemon meringue pie fresh out of the refrigerator. The market forces will drive this implant into most plastic surgeon’s offices. As is typical with new procedures, media hype regarding ‘the latest in breast implants’ may result in some very disappointed patients. While the 410 is a warm welcome to our implant armamentarium it is not the panacea and has many of its own trade offs as well as some particular risks that are separate from regular smooth round gel implants.
